Money Follows the Person Program (MFP)

Money Follows the Person Program (MFP) helps people who are Medicaid-eligible and currently living in a nursing home or other long-term care facility to transition to the community. MFP is a federal demonstration program administered by the CT Department of Social Services.

How Long Does Money Follows the Person Take?

While the goal of MFP is to help move people back into the community as quickly as possible, each transition plan is based on individual needs. Therefore, each timeline to transition is different. The Transition Coordinator will advise individuals about what to expect in their particular situation. Generally, MFP covers services for up to 365 days to successfully transition a person into the community. After the duration, the individual would be directed to another service.

To participate in the program, an individual must:

• Be living in a long-term care facility or hospital setting for at least 60 consecutive days and active on Medicaid (by the time they transition out)
• Have an interest in living in the community
• Be eligible for one of the following CT Medicaid Home & Community-Based Services: Medicaid HCBS Waivers, Community First Choice or State Plan Services (such as nursing, home health aides, occupational therapy and physical therapy).

Based on the individual’s needs, MFP provides the Services & Supports needed for a successful transition into the community.

An MFP Transition Coordinator will serve as an advocate in the transition process. They will work with the individual and their care team on a one-to-one basis to develop the best plan for the individual and may assign emergency back-up services, provide educational resources and help secure a place to live. In addition to returning to an individual’s own home, housing options include individual apartments, assisted living and group homes.

MFP pays for person’s care in the community as well as various expenses that arise when a person moves into a more independent situation. Based on an individual’s circumstances and needs, they may be eligible to receive help with home modifications, rent and security deposits, along with one-time setup costs for a new apartment.
