Money Follows the Person Program (MFP) helps people who are Medicaid-eligible and currently living in a nursing home or other long-term care facility to transition to the community. MFP is a federal demonstration program administered by the CT Department of Social Services.
How Long Does Money Follows the Person Take?
While the goal of MFP is to help move people back into the community as quickly as possible, each transition plan is based on individual needs. Therefore, each timeline to transition is different. The Transition Coordinator will advise individuals about what to expect in their particular situation. Generally, MFP covers services for up to 365 days to successfully transition a person into the community. After the duration, the individual would be directed to another service.
