Home health care helps people recover from illness or injury and remain in their home by bringing services and supports into the home. Prescribed by an individual’s physician, home health care is part of a written plan of care.
While it can include non-medical services, the primary focus is medical and includes rehabilitation services to improve the person’s condition and help them regain their independence. This service often follows hospitalization, rehab or skilled care in a nursing facility.
Home health care can be provided in tandem with other types of home care. Non-medical home care includes help with Activities of Daily Living (ADLs) such as transportation, meal preparation or getting dressed.
