This program provides information about options for home-based care.

You have choices! And this program assists those at risk for a stay in a long-term care facility with information about home and community-based services—even if they are not yet Medicaid eligible.

My Care Options is a new program in Connecticut that provides information about home-based care and support to people who are receiving care in a nursing facility. My Care Options helps those who wish to return home or to the community to fully understand their choices by offering resources, information, and support about options.

What can My Care Options do for you? Services include:

  • Offering care management transition services to move back to the community from a nursing facility—this means a Care Coordinator will meet with you to talk about your options and how to get back home
  • Reviewing your financial information to help figure out if you’re eligible for Medicaid
  • Creating a care plan to meet your specific needs
  • Working with you to hire homecare staff
  • Helping you to understand how to modify your home to best meet your needs and changing abilities

Who are Care Coordinators?

My Care Options Care Coordinators are trusted, trained  professionals. Their goal is to work with you and the nursing home staff to help you safely and successfully transition back into the community if you choose to do so.

Who is eligible?

To participate in the program, you must meet the following requirements:

  • Currently reside in a nursing home or long-term care facility
  • Have an interest in understanding the different care options available to you
  • Be a Medicare Savings Program participant
