Below is a list of the major programs and services in Connecticut. This select group of programs and services has both a high degree of participation and is available statewide for people who are eligible.
HUSKY Health and Medicaid: This program provides a comprehensive health care benefit package including preventive care, primary care, specialist visits, hospital care, behavioral health services, dental services, hospice and prescription medications to eligible individuals.
Community First Choice (CFC): Community First Choice (CFC) offers a variety of services and supports to help individuals meet their goals and to provide hands-on care and supervision in their home and community while self-directing staff.
Money Follows the Person Program (MFP): This federal demonstration program helps individuals living in long-term care facilities and on Medicaid transition back into the community. A wide range of services and supports are available.
CT Home Care Program for Elders (CHCPE): This program enables older adults to remain in their homes with services and supports instead of residing in a nursing facility or other institutional setting to receive care.
Medicare: This is the federal health insurance program for older adults, people with disabilities and people with End-Stage Renal (Kidney) Disease and ALS. Benefits can be received through different programs and policies either from the government or a private insurance company.
