The Edward Smith Library is one of two public libraries serving the Town of North Branford. Situated in the center of Northford, the library is easily accessible at the Intersections of Routes 17 and 22. Teresa Holabird, the Adult Services Librarian at the Edward Smith Library, comments, “We are excited to serve as a location where older adults, people with disabilities, and their families can receive help navigating the resources needed to live independently and successfully.”
She continues, “I have worked here long enough to remember doing actual reference work (Before Computers- gasp!) and I look forward to utilizing this helpful tool to help people find the answers they need. Manchester Housing Authority

The Manchester Housing Authority was created to develop and operate housing and housing programs for low-income families in the Town of Manchester, CT. The agency’s mission includes promoting personal, economic and social upward mobility to provide families the opportunity to make the transition from subsidized to nonsubsidized housing.
The Manchester Housing Authority has joined the My Place CT Partner network to support its efforts to encourage self-sufficiency while addressing educational, socio-economic, recreational and other human services needs of its residents. The Housing Authority works in conjunction with the Town Human Services Department, state agencies, community-based organizations and service providers to support individuals in their desire to live safely and successfully at home.
According to Mary Ann Murray, Director, Resident Services & Congregate Housing, “The My Place CT Partner network is an invaluable resource to enable individuals to meet the needs of aging or disabilities while empowering them to make the best choices from resources available for their lifestyle and goals. The role of the My Place CT Partner is most successful when we listen, reflect back, and assist individuals in exploring their options for optimal success to live in the place they call home.”
Hartford HealthCare Center for Healthy Aging

The Hartford HealthCare Center for Healthy Aging, part of Hartford HealthCare Senior Services, offers a broad spectrum of services including free information, assessments and referrals to community resources with the goal of maximizing the lives of seniors and their caregivers. The Center’s service lines include resource coordination, dementia specialist, transition care nursing and geriatric care management.
The Center helps older adults who want to remain as healthy and independent as possible in the most appropriate environment. The Center also helps connect families and caregivers with resources and assistance that can support their loved ones.
The Center for Healthy Aging joined the My Place CT Partner network as part of the State of Connecticut Department of Social Services Nursing Home Diversification grant.
