Welcome to the new My Place CT Partner network. Your participation is vital to helping residents in our state find what they need to live independently in the place they call home.
More than ever, people want to maintain quality of life as they age or live with a disability. But so few know what to do or where to turn for the help they need.
That’s why Connecticut’s goal is to have at least one place in each town where people can get one-on-one help finding programs and services. As the first Partners in this effort, you are on the front lines making it easier for older adults, people with disabilities and their families to plan, find and manage the care, services and supports they need to live independently in the community.
This first issue of the quarterly FrontLines newsletter will outline the many My Place CT initiatives and introduce you to fellow Partners. Future issues will continue to provide information on available tools and inspiration from the network.
